Uneven dangerous surface

The pavement is u even and cracked and is unsuitable for people with mobility problems. It is a hazard and my daughter has recently tripped and fallen over it. This is after my mother tripped and hurt herself at the beginning of the year. I logged this in jan 2019 and was responded to saying it is on the agenda to be fixed in that financial year. It was also changed to ‘fixed’. It has not been fixed and is very much still a problem. It is outside my house which is 66 Lonsdale way LE15 6LR.
