Pavement collapsing a few inches

The pavement has an indentation of a couple of feet wide and a few inches deep which has become noticeable over the last few months. As it is near drainage, concerned it may be collapsing into the structure below? It is positioned outside a shared drive so is driven over regularly. It is collecting water creating a deep puddle and is becoming a trip hazard.
